About us

Perdix is a case management consultant company operated by the CEO and registered nurse Toni Conde, RN, BSN, CCM. She has a range of experience of 25 years in various specialties such as home health, hospice, behavioral health, school nursing, managed care, travel nursing and more recently, inpatient case management. She's a certified case manager with a Bachelor's Degree in Nursing from Arizona State University.

Her Journey and Challenges

Her specialization lies in inpatient case management, a field she has been a part of for the last 9 years. In her early years, she struggled with understanding the ins and outs of inpatient case management. The overwhelming volume of information to learn and manage a caseload during an 8, 10 or 12-hour shift, coupled with life experiences and job challenges, made her realize how precious time can be.

Learning from Experience

Toni offers a culmination of not only basic inpatient case management skills and knowledge but also her real-life experiences and challenges that required critical thinking skills and problem-solving. She navigated through various situations; from insurance barriers, limited resources for patients, knowledge deficits to complex discharges, her experiences are a treasure trove of valuable information.
